診断書について
■発行にかかる費用
治癒証明書、陰性証明書共に1,100円
■発行にかかる時間
検査結果が出た後の診療日にはお出し出来ます。
英文の陰性証明書について
■Application Fees 発行にかかる費用
– ２０００ yen. 治癒証明書、陰性証明書共に2,000円
■Time to issue 発行にかかる時間
Antigen test : during the day
PCR test : after 2 business days
*It may take longer than the above because the required documents differ depending on the destination.
抗原検査の場合は当日のお渡し、PCR検査の方は翌々営業日となります。渡航先によって必要な書類が異なりますため、それより時間をいただく場合がございます。予めご了承ください。
【注意事項】
・外国籍の方の場合、お名前のご記入にあたりパスポートの写しのご提出をお願いいたします。
・渡航国に応じた記載内容、所定書類の有無を予め外務省、大使館等で確認し、所定書類がある場合には持参してください。
・後日、書類不備等で渡航不可が生じた場合において、当院では責任を負いかねますのでご了承ください。
【Note】
・Foreign residents are required to submit a copy of passport.
・Check with the Ministry of Foreign Affairs, embassies, etc. in advance whether or not there are prescribed documents by the country you travel. Please bring them when they are needed.
・Please kindly note that we are not responsible if you are unable to travel due to incomplete documents.
